How to name St. Luke's as a Beneficiary or in a Will Bequest Designation

Beneficiary Designation:

•  You may name St. Luke’s or St. Luke’s Heritage Fund as a primary or contingent beneficiary on a retirement plan (IRA, 401K, 403B, etc), life insurance policy or Transfer on Death designation from a bank or brokerage firm.  You may divide your beneficiary designation between more than one individual and include St. Luke’s as a beneficiary for a percentage of the proceeds, subject to any rules/regulations of your plan.
• Request a change of beneficiary form from your IRA or retirement plan (e.g. 403B, 401K, etc) custodian, your insurance carrier, or your bank or brokerage firm.  Many plans have these forms available online.   
• You will probably be able to designate a specified amount or percentage of the account to go to St. Luke’s, subject to the rules of the account. 
• You may designate St. Luke’s Episcopal Church or St. Luke’s Heritage Fund as your beneficiary listing.
• You may need to list the Church address: 1737 Hillandale Rd. Durham, NC 27705.  You may also need the Federal Tax ID for St. Luke’s (#56-6033279). 
• After you have filed your change of beneficiary form, be sure to complete the Response Form and give it to Kathy Barnes.

Bequest Language in a Will:

If you decide to name St. Luke’s in your will, here are some language samples for your use.  These are only samples for your consideration.  Your legal counsel can help you to refine the bequest language to suit your specific wishes.  If you do decide to include St. Luke's or St. Luke’s Heritage Fund in your will or beneficiary designation, please complete the Response Form and give to Kathy Barnes.  It will also be very helpful to have a copy of that portion of your will or trust for our records.

• Specific Amount for General Purposes

"I give, advise and bequeath to The Rector, Wardens, and Vestry of St. Luke's Church in Durham North Carolina, the sum of $_____ to be used at their discretion for the general objectives and purposes of said St. Luke's Church".

• Percentage Amount for Specific Purpose

"I give, advise and bequeath to The Rector, Wardens, and Vestry of St. Luke's Church in Durham North Carolina, ___% of the rest, residue, and remainder of my estate to be used at their discretion for (example - the general maintenance of the physical facilities of St. Luke's Church)".

• Unrestricted Bequest to the Heritage Fund

"I give, advise and bequeath to The Rector, Wardens, and Vestry of St. Luke's Church in Durham North Carolina, ____% of the rest, residue, and remainder of my estate, to be placed in the St. Luke's Heritage Fund, for use at their discretion".

• Directed Bequest to the Heritage Fund

"I give, advise and bequeath to The Rector, Wardens, and Vestry of St. Luke's Church in Durham North Carolina, ____% of the rest, residue, and remainder of my estate, to be placed in the St. Luke's Heritage Fund, for use in (example - Mission and Outreach and the Rector's Discretionary Fund)".
